Change your beneficiary

To change your beneficiary, first download and print out the request for change of beneficiary form (form 1813W).

To complete the form:

• Clearly designate the beneficiary by including full name, relationship, date of birth, Social Security number, and address.
• Date and sign as indicated on the form. An adult, other than a named beneficiary, is required to witness the signature.
• Provide a copy of the trust document, if a trust is the designated beneficiary.
• As a reminder, if all beneficiaries are deceased at the time of death, proceeds will be payable according to Royal Neighbors Prior Death bylaws.
