Beneficiary Claims

We have compiled the most frequently asked questions regarding beneficiary claims. If you do not find the answer to your question here, please Contact Us by mail, phone, or e-mail.

How does my beneficiary make a claim for my death benefit?
If you have recently lost a loved one, please accept our sincere sympathy. We would like to help you through this difficult time by offering the convenience of completing our online form which can be used for notification of the death of the insured or annuitant. If you would rather report this death by phone, you may call our Member Services Department at (800) 627-4762. You will need the following information when you report the death: the insured/annuitant's name, the certificate number(s), and the actual date of death. Upon notification, further instructions will be provided.

If I'm a beneficiary, do I have to pay income taxes on a death benefit?
No. Death benefits from life insurance policies are typically free from income tax. However, they may be subject to state or federal estate, and gift taxes. Contact your tax advisor with questions about possible income, estate and gift taxes related to your specific situation.

How can I change my beneficiary designation?
Print out and complete the Request for Change of Beneficiary form (Form 1813W), which is available in the Access Forms section, and mail it to:
