Chapter Roster

A Chapter Roster documents the leaders and members of a chapter. Chapter leaders should be elected during the annual business meeting. Once elected, complete Chapter Roster Form and return it to the Home Office postmarked by March 15. If a change in officers is necessary prior to the annual business meeting, a special election of officers may be conducted at the next business meeting.  

Important information about completing the form:

  • Must be completed and returned to the Home Office postmarked by March 15
  • Required annually for every chapter to maintain active status
  • An adult chapter must have three officers to qualify as an active chapter
  • A youth chapter must have two officers to qualify as an active chapter
  • An individual may not hold more than one office
  • A background check is required for all new youth officers (please contact your Fraternal Development Specialist)

Financial Statement

The Financial Statement is a required form and is used to record your chapter’s financial position on an annual basis.

Important information about completing the form:

  • Must be completed and returned to the Home Office postmarked by March 15
  • Required annually for every chapter to maintain active status
  • A copy of the chapter’s December bank statement must accompany the Financial Statement

Request for 990-N Info

All adult and youth chapters must complete the Request for 990-N IRS Information annually to maintain active and tax-exempt status.   

Important information about completing the form:

  • Must be completed and returned to the Home Office postmarked by March 15 
  • Required annually for every chapter to maintain active and tax-exempt status
  • All three sections must be completed

Fraternal Community Contributions

All adult and youth chapters must complete the Fraternal Community Contributions annually to maintain active status. The questions included in this form are a portion of an annual survey conducted by the American Fraternal Alliance.

  • Required annually for every chapter to maintain active status

  • To be considered for one of the top three $100 awards, a photo must accompany this form postmarked by May 25.

Nomination for Fraternalist of the Year

All adult and youth chapters are encouraged to submit a nomination for Fraternalist of the Year.

  • Nominee must be a member of Royal Neighbors of America
  • Multiple nominees per chapter are allowed
  • Form must be returned to the Home Office postmarked by June 1

All adult and youth chapters must complete a Make A Difference Day Report.  

Important information about completing the form:

  • Must be completed and returned to the Home Office postmarked by Nov 11 to receive the $25 participation bonus
  • To be considered for one of the top three $100 awards, a photo must accompany this form postmarked by Nov 11.

Quarterly Chapter Report

These reports are used to determine the chapter’s eligibility to receive the Royal, Regal, or Crown level of quarterly funding. All adult and youth chapters are required to complete a Quarterly Chapter Report for each quarter of the calendar year. Download and print out the Quarterly Chapter Report.

Important information about completing the form:

  • Due dates:

   - First quarter report (Jan./Feb./Mar.) -- Due by April 15

   - Second quarter report (Apr./May/June) -- Due by July 15

   - Third quarter report (July/Aug./Sept.) -- Due by Oct. 15

   - Fourth quarter report (Oct./Nov./Dec.) -- Due by Jan. 15

  • Required quarterly for every chapter to receive funding and to maintain active status (regardless of activity level)

This form is used to track deposits and withdrawals within the chapter’s checking account. A Record of Chapter Funds is required to be submitted with each Quarterly Report.

  • Required by all adult and youth chapters
  • Must accompany each Quarterly Chapter Report to maintain active status

To request invitations for events, matching funds projects, large community service projects, or fund-raisers, complete Request for Invitation Service and submit to the Home Office.

  • Complete and submit at least 45 days prior to the event or project
  • Provide as much detail as possible

Request for Souvenirs/Door Prize

Distributing Royal Neighbors souvenirs and door prizes at your events and projects is a great way to thank participants while promoting the Royal Neighbors brand. Complete the optional Request for Souvenirs and Door Prize form to request souvenirs and door prizes on a quarterly basis and submit to the Home Office.

Important information about completing the form:

  • Complete and submit at least 45 days prior to the event or project
  • A maximum of 50 souvenirs (adult or youth, or combination) may be requested per quarter
  • One adult door prize may be requested per quarter
  • Contact your Fraternal Development Specialist to request items for large events and projects

Matching Funds Request

This form is required when requesting approval of a fund-raising project where funds will be matched by Royal Neighbors of America. Complete and submit the Request for Matching Funds Project to the Home Office for pre-approval.

Important information about completing the form:

  • Complete and submit at least 45 days prior to the project
  • No commitments should be made until approval is received from the Home Office
  • Royal Neighbors retains the sole and complete discretion to determine whether or not to contribute to matching funds projects

Matching Funds Report

After a matching funds project has been approved and the event has occurred, complete this report to receive matching dollars from Royal Neighbors.  Download and print out the Matching Funds Report.

Important information about completing the form:

  • Matching funds projects must be approved by the Home Office at least 45 days prior to the project
  • Royal Neighbors will match up to $1,000 of funds raised by a chapter during each calendar year
  • A chapter check payable to Royal Neighbors of America for the entire net amount raised by the project must accompany this form
  • Royal Neighbors retains the sole and complete discretion to determine whether or not to contribute to the matching funds project
  • Record this project as a community service project on the Quarterly Report

Request for Teddy Bears

The Teddy Bear Program was designed to provide Royal Neighbors Teddy bears to agencies and organizations that provide assistance to children in stressful situations (i.e. homeless shelters, children’s homes, ambulance services, police/sheriff’s departments, hospitals, pediatric units, and victims of natural disasters). Complete and submit the Request for Teddy Bear Program to the Home Office.

Important information about completing the form:

  • Complete and submit at least 45 days prior to the presentation to receive 10 free Teddy bears
  • A street address is required for shipping
  • Additional Teddy bears may be purchased for $7 each

   - A chapter check made payable to Royal Neighbors of America is required for additional bears

Photographic and Personal Statement Release

When taking photos at your events and projects, please ask the individuals appearing in the photos to sign a Photographic and Personal Statement Release and submit this form with your photos to the Home Office. This form may be downloaded and printed as needed.

Release and Waiver of Liability

If your chapter is hosting an event or project where volunteers are participating in physical labor or activities, please ask each volunteer to sign a Release and Waiver of Liability. This form also includes a photography/audio release and may be downloaded and printed as needed. Please submit the forms to the Home Office upon the completion of your project.
